Supports for Community Living

We are a Supports for Community Living Company who has locations in both Winchester, KY and Jackson, KY. The Supports for Community Living (SCL) waiver provides Medicaid-paid services to adults  with intellectual or developmental disabilities. These supports allow participants to live at home rather than in an institutional setting. Lifetime Potentials is dedicated to providing outstanding care and support to help each and every one of our participants achieve their personal dreams and become active members of our community. 

 Each location has several residential homes and a Day Training Center. Participant's enjoy time in the community volunteering, shopping, going to the library, and many other cultural, educational, and leisure activities. They enjoy taking mini-vacations to places such as Newport Aquarium, Tennessee, and Myrtle Beach. 

Lifetime Potentials also provides Private care services to those not on the waiver in their own home. Our Private care workers provide the same excellent care and support to help your loved one live life to the fullest.

Lifetime Potentials INC. fully recognizes each participant has the right to take a leading role in their life. Lifetime Potentials staff and management will support and empower each participant to take an active part in the decision making processes of their life including but not limited to team meetings, goals, activities, and everyday choices. Lifetime Potentials will support each participant's right to make informed decisions about their lives and their freedom of choice.     

Lifetime Potentials will support and assist participants in remaining connected with natural support networks by assisting in and encouraging communication as appropriate. Lifetime potentials will encourage the formation of new natural supports by assisting the participant to be active in their community forming new relationships and support. 

Lifetime Potentials will promote dignity and self-worth through respect, activity, choice, and encouragement. Lifetime Potentials will help each participant to identify their own unique abilities and maximize their potential in order to promote pride and self-worth. 

Lifetime Potentials will support team meetings and help to ensure that they are person centered and promote the participants right to choice, inclusion, employment, growth, and privacy. Lifetime Potentials will take an active role in advocating for and promoting self-advocacy of the participant during team meetings. 

Lifetime Potentials will provide a restraint-free environment. All use of physical restraint, chemical restraint, seclusion, or aversive techniques shall be prohibited. 

Lifetime Potentials fully supports the SCL program goal that all participants receive person centered waiver services; are safe, healthy, and respected in the participants community; live in the community with effective, individualized assistance, and enjoy living and working in the participants community. 

Lifetime Potentials will assist and support the participant in living the life they choose to the fullest.
